2.	Mail payment directly to Care Lodge. Please include:
		Check or money order made out to Care Lodge with "conference" noted in memo section.
		Your name, address, email and phone.
		Your agency/position, if applicable.
3. At the conference. (Note fee below.) *Registrations will not be processed without payment.

Registration Fee

The fee includes all conference materials, continental breakfast and lunch. We apologize that we will not be able to accommodate special dietary restrictions. Cancellation policy: No refunds will be made for cancellations received after 10/14/10.

Early bird registration	$35.00
For registrations received with payment by 9/30/10.

Regular registration	$50.00
For registrations received with payment by 10/14/10.

Late registration (or at door)	$75.00*
For registrations/payments received 10/15/10 or after.
*Will not include materials or lunch.
